4. Click the “Submit” button
5. Check your email
6. Get promotion code
7. Scroll down in email to your next steps

9. Print form that pops up after step #8
10. Fill out form & input promotion code where applicable
11. Attach whitening gel lot number & receipt to form
12. Mail your cut-out lot number, dated receipt, and form to:
